Service Coordinator

The Service Coordinator will coordinate individualised care and resources to consumers, you will schedule services for our clients depending on the support package they have purchased.

Your key responsibilities include:

  • Co-ordinating appropriate care and services for clients in consultation with external agencies.
  • Sourcing and booking all care shifts including but not limited to, Personal Care, Homecare, Gardening, Maintenance etc.
  • Managing all services or shift cancellations and rescheduling as required.
  • Consulting with clients, family members, Care Managers, external agencies and other supports to ensure that care is ongoing and that all client needs are identified and met.
  • Liaising with Community Care Providers and Case Managers of brokered services to ensure an appropriate understanding of client needs.

Field Staff Team Leader

Work closely with our field staff to ensure that the delivery of client centric services is in line with the vision and values of the organisation, producing quality care. Lead and motivate frontline workers, mentoring and coaching, supporting developing opportunities and providing exceptional customer service.

Your key responsibilities include:

  • Establish and maintain positive relationships with internal and external stakeholders; including office and field staff as well as clients, families and guardians.
  • Provide ongoing guidance, mentoring and training to a team of community support workers.
  • Complete documentation and administrative reporting as required.
  • Assisting in the successful process of recruitment, onboarding, and induction of new staff to achieve client and business objectives.
  • Manage and lead the day-to-day operations of your team including the recruitment, training, and induction of our support workers and nurses.
  • Manage client service feedback and complaints and ensure feedback process is addressed and complaints resolved.
