This position is to support clients on our TRANSPAC program, a 12-week package that assists with transitioning clients from hospital back to living as independently as possible in their own homes. The services you will assist with include home exercise programs, personal care, domestic assistance and social support.

To be successful you will need:
Current Certificate III in Aged Care or equivalent
Current First Aid and CPR certificates
Current NDIS check
The right to work in Australia
Experience working in aged, disability care or domestic assistance is desirable, but not required.
