Care Manager

As a Care Manager you will work on the delivery of best practice client care in collaboration with our care management team. This dynamic and important role within the myHomecare Group is to ensure and maintain a high standard of care, to enhance our values and support our clients.

Your key responsibilities include:

  • Manage client service delivery across their support package. 
  • Develop Care Plans aligned with the client’s needs and goals in conjunction with the Aged Care Assessment Team (ACAT) assessment.
  • Co-ordinate multidisciplinary teams to deliver the best care approach.
  • Ensure compliance with all relevant legislative and industry standards.

Clinical Care Manager

As a Clinical Care Manager you are to provide clinical guidance and clinical nursing services to clients through assessment and establishment of  treatment plans in consultation with the client, within their own environment, with a focus on wellness and re-ablement, to encourage the client to maintain their independence for as long as possible. 

Your key responsibilities include:

  • Deliver excellent client care for older Australians.
  • Manage and oversee a high standard of care provided by the myHomecare Group providers and staff.
  • Use your experience, customer service and aged care management skills to make a mark on the community. In doing so, you’ll help keep ageing Australians safe at home for longer.
  • Develop, monitor, and reassess Care Plans in consultation with clients, their families and other healthcare professionals.
  • Undertake and complete comprehensive clinical assessments.
